Healthcare Patient Portal is a web & mobile application project built by ZTABS over 7 months. The challenge: [object Object] Key results: 74% scheduling workload reduction, 22% to 9% no-show rate, 60% in 90 Days patient adoption.
The clinic network relied on manual phone scheduling that consumed 30+ staff hours per week. Patient records were siloed across locations, making it difficult for providers to access complete medical histories. Billing discrepancies and no-show rates were cutting into revenue.
We built a cross-platform application using React Native for mobile and Next.js for web, backed by a PostgreSQL database with row-level encryption for PHI compliance. The scheduling module integrates directly with each clinic's provider calendar and sends automated reminders via SMS and email. Telehealth visits use end-to-end encrypted WebRTC connections. Payment processing is handled through a PCI-compliant Stripe integration with automated insurance co-pay calculations.
The portal reduced phone-based scheduling workload by 74% and cut patient no-show rates from 22% to 9%. Over 60% of registered patients adopted the platform within the first 90 days of launch across all 12 clinic locations.

The real options on the table before we picked this stack for Healthcare Patient Portal.
| Alternative | Best For | Cost Signal | Biggest Gotcha |
|---|---|---|---|
| Epic MyChart (enterprise EHR portal) | Health systems already on Epic with budget for enterprise licensing | $1.2M-$500M+ multi-year Epic contract | Customisation ceiling is real; regional clinics rarely see their workflows prioritised. |
| Athenahealth or eClinicalWorks portals | Mid-market multi-specialty practices already using their EHR | $140-$800/provider/mo | Telehealth, scheduling, and payments are siloed across vendor add-ons; UX inconsistency hurts adoption. |
| Redox + custom portal | Teams that want to integrate with multiple EHRs via a single API layer | $3k-$15k/mo Redox + build cost | Redox usage fees scale with volume; cost becomes meaningful past 50k monthly messages. |
| Custom React Native + Node.js (our build) | Clinic networks wanting unified scheduling, telehealth, records, and payments in one HIPAA-compliant app | ~$300k-$450k build + $5k-$11k/mo HIPAA ops | HIPAA BAAs, audit logs, and encryption-at-rest add real ongoing cost; every vendor change requires BAA review. |
Specific numbers — build cost, ops, and the scale at which custom beat the off-the-shelf alternative.
The patient portal cost ~$380k to build + ~$7k/mo HIPAA-compliant ops. Epic MyChart was prohibitive for a 12-clinic network, but Athenahealth’s portal stack plus telehealth add-ons was quoted at ~$320k/yr across 60 providers. The custom build broke even at month 16 on licence delta alone, earlier when patient adoption (scheduling-online conversion rose from 18% to 61%) lifted fill rates. Below 4 clinics reselling an EHR portal still wins; above 20 clinics the custom portal saves ~$180k/yr and gives the network a differentiated patient experience.
Specific production issues this pattern has surfaced for real teams.
Patients registered at multiple clinics with slight name or DOB differences created duplicate records; a fuzzy-matching merge flow with clinic staff review resolved duplicates without losing history.
End-to-end encrypted WebRTC video degraded in rural broadband conditions; an adaptive bitrate fallback plus an audio-only option preserved the visit when video failed.
Some insurance plans calculated co-pays differently for telehealth vs. in-person visits; a plan-specific rules table had to be added rather than hardcoding a single co-pay formula.
We say this out loud because lying to close a lead always backfires.
The multi-clinic architecture and role model introduce overhead a solo practice does not need, where a lightweight scheduling tool plus a telehealth add-on suffices.
Large hospital systems on Epic or Cerner need full-depth HL7/FHIR interoperability beyond the portal's integration surface.
The compliance model centers on HIPAA; GDPR-only or PIPEDA jurisdictions need adjusted lawful-basis and consent flows before deployment.
